Let's get started.

Thanks for taking the time to fill out our intake form! This form is designed to help us identify the most prominent growth areas for our process together. Try to respond as efficiently as you can—shoot to only spend a minute or two on each question, we don't need polished language, just a sense for what feels easy and clear,  and more importantly what doesn't. It should only take about 30 minutes to complete. 

NOTE: If you need to stop before you've finished, just make a note of where you leave off and submit the partial form.
When you have time to fill in the rest, just reenter your name and email so we know it's you.

Customer: Describe your ideal customer profile (ICP).

Problem: What specific problem are you solving for your customer? Please describe in detail.

Process & Methodology: How do you do things differently from your competition?
